Athletic Participation/Extra-Curricular Physical Requirements for 2020-2021

 

Dear Parents, please note the following information and requirements for athletic and/or extra-curricular participation for the 2020-2021 school year for those that are required to have a physical.

 

It is SISD school board policy to have a new physical each year that your son/daughter participates in UIL athletics/extra-curricular activities.  However, due to COVID-19 the SISD school board voted to accept the 2019-20 UIL physical for the 2020-21 school year only.  While we still suggest getting a physical this year, it will not be mandatory if your child turned in a UIL physical in 2019-20.

Who requires a physical for this year?

Incoming 7th graders, those new to the district and/or those that did not participate in UIL activities during the 2019-20 school year will be required to get a physical for the 2020-21 school year.  Please see below for the other requirements for UIL athletic participation/extra-curricular activity.

  1. Signed Medical History Form (Deadline – August 17th) - While a physical is not required for this year if one was turned in last year, a signed medical history portion of the UIL physical is STILL required for both in-person and virtual workouts.This needs to be filled out, signed by the athlete/extra-curricular participant and their parent/legal guardian.Click the link HERE to be redirected to the web page.Scroll down and select the Physical Form 2020-2021 link.

    Parent Instructions:

    1. Please print the medical history document and write your child’s ID number at the top.
    2. Complete ALL questions AND include a current phone number in case we need to contact you.
    3. Sign the document and have your child sign as well where indicated at the bottom of the form.
    4. Scan to your computer using a scanner or preferably take a high-quality scan on your cell phone using the “Notes” application.
    5. If you are an athlete, please email the scanned document to your athletic trainer.
    6. Other extra-curricular activities, please scan/email to the appropriate sponsor/director.

       

  2. Skyward Signatures for all UIL documents (concussion, cardiac arrest, steroid, extracurricular participation, bullying, insurance, and athlete code of conduct).The Deadline is August 17th in order to participate in athletics/extra-curricular activities virtually for the first day of school.Parent’s will need to login in to Skyward using the “Parent’s login and password” (not the students).If the parent has forgotten their username/password, then they will need to contact the school’s PEIMs Clerk.

    1. Open your child’s registration.
    2. Complete all steps making sure you electronically sign all UIL athletic participation documents.
    3. For athletes only please make sure you IDENTIFY AND MARK which UIL sport(s) your child is participating in.
    4. Save.After it is saved it will be automatically updated.

     

  3. SISD Medical Authorization Forms - If your child uses an asthma inhaler, an epi-pen, has diabetes, or other condition that requires Physician’s orders, they will need to have their physician fill out and sign the appropriate SISD form before they can practice or participate in any in-person or virtual practices/games.Also, all asthma inhalers, epi-pens, or other necessary medication needs to be current and checked in with the athletic trainer or your appropriate sponsor/director before participating in any in-person athletic/extra-curricular activity that requires a physical/medical history form.
